Job Description
We are seeking a passionate and dedicated NDIS Support Planner to join our team in Gladstone, QLD. This role is pivotal in assisting participants and their families to navigate the National Disability Insurance Scheme (NDIS) and develop personalised support plans that align with their goals and aspirations. You will work closely with individuals to understand their needs, connect them with appropriate services, and ensure their NDIS plans are effectively implemented and reviewed. Key Responsibilities
- Facilitate person-centred planning meetings with NDIS participants and their families/carers.
- Assist participants in understanding their NDIS plans and allocated budgets.
- Develop, review, and adjust individualised support plans based on participant goals and changing needs.
- Connect participants with suitable service providers and community resources.
- Monitor the implementation of support plans and ensure services are delivered effectively.
- Maintain accurate and confidential participant records in line with NDIS requirements.
- Advocate for participants to ensure their rights and choices are upheld.
- Provide information and guidance on NDIS policies, procedures, and funding guidelines.
- Collaborate with internal teams and external stakeholders to achieve positive participant outcomes.
Required Skills
- Minimum 2 years of experience in the disability sector or a related human services field.
- Strong understanding of the National Disability Insurance Scheme (NDIS) framework and principles.
- Excellent communication, interpersonal, and negotiation skills.
- Demonstrated ability to build rapport and work collaboratively with diverse individuals.
- Proficiency in using case management software and Microsoft Office Suite.
- Ability to work independently and as part of a multidisciplinary team.
- Valid Australian driver's license and access to a reliable vehicle.
- Current NDIS Worker Screening Check or willingness to obtain one.
- First Aid and CPR Certificate or willingness to obtain one.
Preferred Qualifications
- Tertiary qualification in Disability, Social Work, Allied Health, or a related field.
- Experience in NDIS plan management or support coordination.
- Knowledge of local community resources and service providers in the Gladstone region.
