Job Description
We are seeking a dedicated and skilled Community Health Worker to join our dynamic team in Bendigo. This vital role involves working directly with community members to assess their needs, provide health education, and connect them with essential health and social services. You will be instrumental in empowering individuals to manage their health proactively, navigate complex systems, and improve their overall quality of life within a not-for-profit framework. This is a front-line position where your compassion, expertise, and commitment will make a tangible difference every day.
Key Responsibilities
- Conduct community outreach and engagement activities to identify individuals and families in need of support.
- Perform needs assessments to understand clients' health, social, and cultural requirements.
- Provide culturally sensitive health education on a range of topics including chronic disease management, preventive care, and healthy lifestyle choices.
- Connect clients with appropriate internal and external health, social, and welfare services and advocate on their behalf.
- Develop and maintain strong working relationships with community organisations, health providers, and government agencies.
- Maintain accurate and confidential client records, documenting all interactions and service referrals.
- Contribute to the development and evaluation of community health programs and initiatives.
- Work collaboratively within a multidisciplinary team to ensure holistic client care.
Required Skills
- Minimum of Certificate IV in Community Services, Health Services, or a related field.
- Demonstrated experience in community work, health promotion, or social support roles.
- Excellent interpersonal and communication skills, with the ability to build rapport with diverse community members.
- Strong understanding of the social determinants of health and their impact on community wellbeing.
- Ability to work independently and as part of a team, managing a diverse caseload.
- Proficiency in basic computer applications and record-keeping.
- Valid Australian Driver's License and access to a reliable vehicle.
Preferred Qualifications
- Previous experience working within the not-for-profit sector.
- Knowledge of local health and community services available in the Bendigo region.
- First Aid and CPR certification.
- Fluency in a language other than English relevant to Bendigo's diverse community.
