Job Description

We are seeking a dedicated and experienced Community Health Worker to join our Not-for-Profit team in Devonport, Tasmania. In this skilled role, you will be instrumental in empowering individuals and communities to improve their health and wellbeing. You will work directly with diverse populations, providing support, information, and advocacy, fostering connections to essential health and social services. This position requires a proactive, empathetic, and culturally sensitive individual with a strong understanding of community health principles and local resources.

Key Responsibilities

  • Conduct needs assessments and develop individual care plans for community members.
  • Provide health education, information, and outreach on various health topics.
  • Connect individuals with appropriate health, social, and welfare services.
  • Advocate on behalf of clients to ensure access to necessary resources and support.
  • Facilitate group workshops and community-based health promotion activities.
  • Maintain accurate and confidential client records in line with organisational policies.
  • Collaborate effectively with internal teams and external agencies to ensure holistic support.
  • Identify and report on community health trends and service gaps.

Required Skills

  • Demonstrated experience as a Community Health Worker or similar role.
  • Excellent interpersonal and communication skills, both written and verbal.
  • Strong cultural competency and ability to work with diverse populations.
  • Proficiency in case management and client advocacy.
  • Ability to work independently and as part of a multidisciplinary team.
  • Sound knowledge of community health principles and local support services.
  • Current Driver's License and access to a reliable vehicle.
  • First Aid certification.

Preferred Qualifications

  • Diploma or Certificate IV in Community Services, Health Promotion, or a related field.
  • Experience working within a Not-for-Profit organisation.
  • Familiarity with local Devonport and Tasmanian health service networks.
  • Bilingual abilities.
