Job Description
Join our dedicated team at SA Health in Port Lincoln as a Palliative Care Support worker. This rewarding role involves providing compassionate and respectful care to patients requiring palliative support, helping to enhance their quality of life, comfort, and dignity. You will work closely with patients and their families, as well as a multidisciplinary team of healthcare professionals, to ensure holistic and person-centred care is delivered. Key Responsibilities
- Provide compassionate and person-centred care to palliative care patients, respecting their choices and cultural backgrounds.
- Assist patients with daily living activities, personal care, mobility, and feeding, always ensuring their comfort and dignity.
- Monitor and accurately report any changes in patient condition, behaviour, or needs to the nursing and medical team.
- Offer emotional support to patients and their families, maintaining professional boundaries and sensitivity.
- Maintain accurate, confidential, and timely patient records in accordance with SA Health policies and procedures.
- Collaborate effectively with nurses, doctors, allied health professionals, and other support staff as part of a multidisciplinary team.
- Adhere to all health and safety protocols, infection control guidelines, and individual patient care plans.
- Contribute to creating a calm, supportive, and respectful environment for patients and their visitors.
- Participate in team meetings and professional development activities as required.
Required Skills
- Certificate III in Individual Support (or equivalent qualification in Aged Care, Disability, or Home and Community Care).
- Demonstrated empathy, compassion, and sensitivity towards individuals with complex care needs.
- Excellent verbal and written communication skills with the ability to build rapport with patients, families, and colleagues.
- Ability to work effectively both independently and as a valued member of a multidisciplinary team.
- Basic computer literacy for record-keeping and communication.
- Current First Aid and Cardiopulmonary Resuscitation (CPR) certificates.
- Ability to obtain a satisfactory National Police Check and Working With Children Check.
Preferred Qualifications
- Previous experience in a palliative care, aged care, or acute care setting.
- Valid Australian Driver's License and access to reliable transport.
- Experience with electronic health records systems.
- Further education or training in palliative care or end-of-life care principles.
