Job Description
We are seeking an experienced and dedicated NDIS Support Coordinator – Specialist to join our team in Cairns. This pivotal role involves providing high-level support coordination to participants with complex NDIS plans, ensuring they receive the appropriate supports and services to achieve their goals. You will navigate the NDIS framework, advocate for participants, and build their capacity to manage their own plans, always focusing on their individual needs and aspirations.
Key Responsibilities
- Provide specialist support coordination for NDIS participants with complex needs, including those with challenging behaviours or intricate service requirements.
- Assist participants in understanding and implementing their NDIS plans, connecting them with suitable service providers and community resources.
- Build the capacity of participants and their families/carers to exercise choice and control over their supports.
- Proactively identify and address potential barriers to plan implementation, providing advocacy and mediation where necessary.
- Monitor and review participant progress, ensuring services remain aligned with goals and NDIS requirements.
- Prepare comprehensive reports and maintain accurate records in compliance with NDIS guidelines and internal policies.
- Collaborate with internal teams and external stakeholders to ensure integrated and holistic support.
- Provide crisis resolution and risk management strategies for participants when required.
Required Skills
- In-depth knowledge of the NDIS framework, policies, and guidelines.
- Demonstrated experience in complex case management within the disability sector.
- Exceptional communication, interpersonal, and negotiation skills.
- Strong problem-solving abilities and a proactive approach to challenges.
- Ability to work autonomously and manage a diverse caseload effectively.
- Proficiency in documentation and report writing.
- Tertiary qualification in Social Work, Allied Health, Human Services, or a related field.
Preferred Qualifications
- Previous experience working with a diverse range of disabilities and complex support needs.
- Postgraduate qualifications in a relevant field.
- Current First Aid and CPR certificates.
- Valid Australian Driver's Licence and access to a reliable vehicle.
