Job Description
Join our dedicated team as a Public Service Welfare – Community Care professional in Port Macquarie. This pivotal role involves providing direct support, advocacy, and resources to vulnerable individuals and families within the community. You will assess client needs, develop tailored support plans, facilitate access to essential services, and foster community engagement to enhance well-being and resilience. Your work will directly contribute to improving the lives of those facing various challenges, from social isolation to financial hardship, within a supportive and collaborative environment.
Key Responsibilities
- Conduct comprehensive needs assessments for individuals and families.
- Develop, implement, and review individualised support plans.
- Provide information, advocacy, and referral services to connect clients with appropriate resources.
- Facilitate workshops and community programs aimed at promoting well-being and skill development.
- Maintain accurate and confidential client records in line with organisational policies and ethical guidelines.
- Collaborate with external agencies, government departments, and community groups to coordinate integrated support.
- Participate in team meetings, supervision, and ongoing professional development.
- Respond to crisis situations and provide immediate support as required.
Required Skills
- Minimum 1 year of experience in community services, social work, or a related field.
- Relevant tertiary qualification in Social Work, Community Services, Welfare, or a related discipline.
- Demonstrated ability to assess client needs and develop effective support plans.
- Strong interpersonal, communication, and active listening skills.
- Sound understanding of welfare principles, ethical practice, and relevant legislation.
- Current Australian Driver's License and access to a reliable vehicle.
- Valid Working With Children Check (WWCC) and National Police Check (or ability to obtain).
Preferred Qualifications
- Experience working with diverse client groups, including Aboriginal and Torres Strait Islander communities.
- Knowledge of local community resources and service networks in the Port Macquarie region.
- First Aid and CPR certification.
- Proficiency in a second language relevant to the local community.
