Job Description

We are seeking a dedicated and compassionate Case Manager to join our team in Taree, NSW. This vital role involves providing comprehensive social work support to individuals, children, and families within the local community who are experiencing vulnerability or hardship. As a Case Manager, you will be responsible for assessing needs, developing and implementing tailored case plans, and coordinating access to essential services and resources. This position requires a professional who is resilient, empathetic, and committed to making a tangible difference in the lives of others, working within a government and not-for-profit framework to uphold the well-being of the community.

Key Responsibilities

  • Conduct comprehensive assessments of client needs, strengths, and risks to develop individualized case plans.
  • Provide ongoing case management, support, and advocacy for clients, ensuring their well-being and safety.
  • Liaise with internal and external stakeholders, including other government agencies, NGOs, and community service providers, to facilitate integrated service delivery.
  • Monitor and review case plans regularly, adjusting interventions as needed to achieve desired outcomes.
  • Maintain accurate and confidential client records and documentation in accordance with departmental policies and legal requirements.
  • Respond to crisis situations with sensitivity and professionalism, implementing appropriate intervention strategies.
  • Empower clients to develop life skills, access resources, and advocate for themselves to achieve greater independence and self-sufficiency.
  • Participate in team meetings, supervision, and professional development activities to enhance practice and contribute to a supportive work environment.

Required Skills

  • Tertiary qualification in Social Work, Psychology, Welfare, or a related field (e.g., Bachelor's or Master's Degree).
  • Demonstrated experience in case management, particularly within government or not-for-profit sectors.
  • Strong understanding of child protection legislation, family violence frameworks, and social welfare policies in NSW.
  • Excellent written and verbal communication skills, with the ability to engage effectively with diverse client groups and stakeholders.
  • Proven ability to conduct needs assessments, develop intervention strategies, and manage complex caseloads.
  • High level of empathy, resilience, and cultural competence, with an understanding of Aboriginal cultural protocols.
  • Proficiency in using case management software and general office applications.

Preferred Qualifications

  • Membership with the Australian Association of Social Workers (AASW).
  • Experience working with Aboriginal communities and an understanding of specific cultural considerations.
  • Current NSW Class C Driver's License and willingness to travel within the region.
  • Post-graduate qualification in a relevant field.
  • Familiarity with local community resources and services in the Taree region.
