Job Description
We are seeking a dedicated and experienced Support Coordinator (Level 2) to join our team in Mackay. In this pivotal role, you will empower NDIS participants to achieve their goals by connecting them with appropriate services and supports, building their capacity to navigate the NDIS, and ensuring they receive planned and coordinated support. You will work with a diverse caseload, advocating for participants and ensuring their NDIS plans are effectively implemented. Key Responsibilities
- Provide high-quality NDIS Support Coordination (Level 2) services to participants, focusing on capacity building and connection to mainstream and community supports.
- Develop and implement individualised support plans in collaboration with participants, their families, and other stakeholders.
- Monitor and review NDIS plans, ensuring services are delivered effectively and participants' goals are being met.
- Navigate complex NDIS frameworks, advocating for participants' rights and needs.
- Build and maintain strong relationships with participants, service providers, and community organisations.
- Assist participants with NDIS plan reviews, appeals, and problem-solving related to their support.
- Maintain accurate and timely records in line with organisational policies and NDIS requirements.
- Collaborate with internal teams to ensure holistic support for participants.
Required Skills
- Proven experience as an NDIS Support Coordinator (Level 2 or similar role).
- Thorough understanding of the NDIS Act, rules, and operational guidelines.
- Exceptional communication, interpersonal, and negotiation skills.
- Strong organisational and time management abilities.
- Capacity to work autonomously and as part of a multidisciplinary team.
- Valid Australian Driver's License and access to a reliable vehicle.
- Current NDIS Worker Screening Check (or willingness to obtain).
Preferred Qualifications
- Tertiary qualification in Disability, Social Work, Allied Health, Community Services, or a related field.
- Experience working with culturally and linguistically diverse (CALD) communities and Aboriginal and Torres Strait Islander peoples.
- Knowledge of local service providers and community resources in the Mackay region.
